You can get them from bottles. The old guy who tells you about them even says you can get them from the bottles. You just don't get a lot.
You often find them on table tops or similar spots where there are Empty Sunset Sarsparilla Bottles around. In houses or in some random shack in the wastes. I've gotten maybe 17, and I don't think I've ever gotten a Star Cap from drinking. I drink a LOT of Sunset Sarparilla too.

You do get them by drinking the sarsparillas, but it's pretty random. something like 1 out of every 15 or 20 bottles gives you a star cap. I had an encounter just outside of Nipton with a guy who had a Lucky Necklace made with those caps. needless to say he didn't get to keep them... been at it for a while and so far I've only got 11 star caps. your best bet is to explore everything.

Way3: Get Sunset Sarsaparilla Star Caps in Fallout New Vegas

They are on the bottles, just drink a bunch of the sarsaparilla and you'll have some eventually.. also if you go to the sarsaparilla headquarters, theres a couple there, plus as you'd expect, load of sarsaparilla so i got like 17 while I was there. However, I have no clue how many you need to turn into festus for a prize.
