Three French surgeons were the first plastic surgeons to presenttheir liposuction experience at the 1982 American Society of Plastic andReconstructive Surgeons annual meeting in Honolulu, Hawaii. The procedure wasinitially faced with criticism in the United States. In late 1982, a “blueribbon committee” was assigned by The American Society of Plastic andReconstructive Surgeons to visit those pioneer plastic surgeons in Paris andthe committee returned with a report that was cautiously optimistic. Americansurgeons’ interest in liposuction has steadily risen ever since, ahead of the publicgrowing demand for minimally invasive body contouring procedures.   More individuals think that they need toundergo liposuction, although liposuction might not be the right procedure fortheir needs. Case selection is a crucial determinant of a perfect aestheticsurgical result, especially in body contouring procedures such as liposuction.Not all individuals who request liposuction are good candidates. A goodconsultation begins with an assessment of your goals. What do you wish tochange about your body? What do you hope to accomplish with a liposuction? Anexperienced plastic surgeon would provide you with a realistic appraisal ofwhat can and cannot be accomplished through liposuction.   Some cases may require alternative therapeuticprocedures such as an abdominoplasty or a liposuction procedure that is combinedwith another open surgical procedure. An astute surgeon is wary of patients whoare considered poor candidates for a successful liposuction procedure such asperfectionists with unreal perceived self deformities, severely depressed individualswho suffer from eating disorders and markedly obese individuals who areincapable of losing weight and expect liposuction to do it for them.   A thorough weight history is an essentialpart of any liposuction consultation. Ideal candidates are at a stable weightwith a successful diet and exercise regimen going on. Patients who have ahistory of marked weight fluctuations are at high risk for weight gain after theprocedure. Maintaining a stable weight and practicing a diet and exerciseregimen for at least 6 to 12 months indicates the necessary commitment tolifestyle change.   Liposuction should not be resorted to asa treatment modality for obesity. In a perfect world, it is used to removegenetically distributed or diet resistant fat. In practical terms, however, itis often used to remove fat that could be lessened with diet and exercise.Ideal liposuction candidates are within 15 to 20 % of their ideal body weightor no more than 55 pounds above chart weight.
